How do you permanently change your username (URL) on Facebook?

I mistakenly changed my personal Facebook profile page to the URL that I wanted for my band's page. I then went through the process of changing the URL to another. However, after I had done this, both the first and second URL led to my personal page, so I cannot use the first for my band page. Help!
